COVID-19 vaccine follow-up survey for faculty, staff and students

The Purdue COVID-19 Vaccine Allocation Task Force is conducting a brief 30-second follow-up survey on the COVID-19 vaccine to help gauge what share of our campus community has been fully or partially vaccinated, where individuals plan to receive the vaccination and how much vaccine supply will be needed over the next month at our campus vaccine site.  

Your participation is critical in helping us plan for the remainder of the spring semester and for what the fall semester will look like on our campus as a majority of our campus becomes vaccinated against COVID-19. This survey closes at 5 p.m. this Friday, April 16. All responses are anonymous.

Getting vaccinated is perhaps the most important step we’ll take as we work together to get back to normal, and Purdue continues to encourage all Boilermakers to get vaccinated as soon as they are able. The more of our campus community who are vaccinated, the more protected we all are and the sooner we can ease current Protect Purdue restrictions.
